Qualifications

• Licensure as RN in the state (as applicable)
• Experience in patient education and care planning preferred
• Ability to assess and respond to patient and family needs with clinical judgment

Responsibilities

• Collaborates in establishing the plan of care and coordinates care delivery across the continuum.
• Assesses pertinent information to determine nursing diagnoses and appropriate interventions.
• Evaluates patient progress toward goals and ensures effective outcomes.
• Promotes health and a safe patient environment.
• Accepts additional duties beyond listed responsibilities as needed.
