• Assess, plan, implement, and evaluate individualized care plans.
• Educate patients and families about care, risks, and treatments.
• Coordinate care with multidisciplinary teams and delegate tasks as needed.
• Document all care activities accurately per policy and regulation.
• Maintain licensure, certifications, and participate in competencies.
• Contribute to process improvements and patient satisfaction initiatives.
• Perform other duties as assigned.
