Job summary

Elara Caring is seeking a Clinical Team Manager – Hospice to lead a compassionate home health team in Missouri. You will oversee patient care services, coordinate care with physicians and facilities, and drive clinical excellence while ensuring timely starts and quality plans of care. Travel up to 50% is required.

You will report to Administration or the Branch Director and manage hiring, coaching, and performance metrics within a supportive, patient-centered environment.

Qualifications

  • Associate degree in nursing or related field; Bachelor preferred.
  • 2 years of home care experience in Home Health or Hospice.
  • 1 year of supervisory experience required.
  • Current, unrestricted RN license valid for the state of work.
  • Able to travel up to 50% as needed.
  • Ability to lift up to 50 pounds and assist with transfers.
  • Valid driver’s license and insurance.

Responsibilities

  • Provides oversight of all patient care services and personnel, including assignments, coordination of patient care, referrals, and individualized patient plans.
  • Plans, directs, and delivers home health services, coordinating with providers and facilities.
  • Ensures timely start of patient care within regulatory guidelines.
  • Coordinates physician orders and communications among care team, patients, and families.
  • Reviews and approves plan of care and evaluates proposed changes for clinical appropriateness.
  • Integrates physician orders and ensures approval by the responsible physician.
  • Conducts regular OASIS reviews for all patients.
  • Hires, interviews, and recommends direct care staff.
  • Communicates with Branch Director/Administration on performance metrics; available for field work.

What you’ll bring as Clinical Team Manager?
  • Provides oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.
  • Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities.
  • Ensures the timely start of patient care within recommended regulatory guidelines.
  • Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate).
  • Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness.
  • Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician.
  • Conducts regular OASIS reviews for all patients.
  • Meets with all direct care team members on a regular basis providing guidance, coaching and communicating information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff.
  • Communicates frequently with the Branch Director or Administrator on key performance metrics.
  • Remains available for field work, as needed.
What is required?
  • Associate degree in a Nursing related field, Bachelor preferred
  • 2 years of home care experience in Home Health or Hospice environment
  • 1 year of supervisory experience required
  • Current, unrestricted RN license valid for the state of work
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
  • Able and willing to travel up to 50% as needed to support business and patient needs.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver’s license and insurance and reliable transportation to perform job tasks
