Job summary

Live Well Homecare is seeking an RN to join an interdisciplinary team in a clinical setting in Augusta, GA. The role focuses on individualized, evidence-based patient care aligned with standards and ethics, with opportunities to mentor others and participate in quality improvement.

The RN will perform comprehensive assessments, coordinate care with the team, and ensure safety and adherence to policies. A nursing degree and active RN license with BLS/PALS are required or preferred.

Qualifications

• Associates or Bachelor's degree in Nursing (preferred).
• Valid RN license (single-state or multi-state compact), BLS certification, and PALS certification or equivalent.
• Experience in direct patient care preferred.
• Strong communication, interpersonal, critical thinking, and time management skills.
• Proficiency in technology and basic computer skills, with the ability to respond effectively in emergency situations.

Responsibilities

• Perform comprehensive nursing assessments, diagnoses, planning, implementation, and evaluation using evidence-based practice and critical thinking.
• Deliver relationship-based, patient-centered care that considers cultural diversity and involves patients and families through effective communication and education.
• Coordinate safe, quality care through interdisciplinary collaboration, timely documentation, and promoting a respectful, inclusive environment.
• Support performance improvement initiatives, participate in research, and uphold organizational policies and safety standards.
• Serve as a preceptor or mentor, fostering a competent and professional nursing workforce.
• Engage in evidence-based practice, research activities, and resource development to improve patient outcomes.
