Job summary

Centra Health seeks an RN Clinical Navigator to provide clinical care and guidance to high‑risk patients, removing barriers to care and coordinating with Centra and external resources to ensure timely access to monitoring and services.

You will build relationships with patients, families, and regional providers, use nursing processes to guide care, and support informed decisions while complying with ANA Code of Ethics and Virginia Nurse Practice Act.

Qualifications

• Must be a graduate of an accredited nursing program.
• BSN degree preferred.
• At least three years of nursing experience and current licensure.
• Current Virginia RN license or multi-state privilege; BLS required; LTSS certification due within 30 days of hire.
• LTSS certification to be obtained within first 30 days and maintained.

Responsibilities

• Provides guidance and education appropriate to patient demographic and clinical needs.
• Collaborates with community outreach programs and resources.
• Connects patients to programs for continued care; empowers patients to manage health.
• Manages programs and resources within clinic; applies disease management in care delivery.
• Ensures timely scheduling of physicians, specialists, diagnostics, and support services.
• Maintains patient information for multidisciplinary teams and community resources.
• Makes referrals to community resources; acts as liaison to care team.
• Assists patients and families in accessing culturally appropriate care; promotes informed decision making.
• Identifies patient problems/needs and helps set priorities for follow-up or referrals.
• Uses nursing process to coordinate patient education needs and engages in community outreach.
• May perform other duties as assigned or updated.
