Dakota Home Care in Fargo is seeking an experienced RN to step into a clinical leadership role as RN Clinical Manager. You will oversee patient care, coordinate nursing services, and mentor our direct-care nurses to ensure safe, high-quality care.

Working with the Director of Nursing, you will ensure regulatory compliance, develop care plans, and advocate for patients throughout the home-care process. The schedule emphasizes work-life balance with daytime hours Monday through Friday.

Job description

Dakota Home Care is seeking an experienced Registered Nurse to join our Fargo team in a clinical leadership role. This position is responsible for overseeing patient care, coordinating nursing services, and supporting our team of direct-care nurses to ensure safe, high-quality care.

Working closely with our Director of Nursing, the RN Clinical Manager will help ensure clinical excellence, regulatory compliance, and continuity of care for the individuals we serve.

Monday–Thursday: 8:00 AM–4:30 PM

Friday: 8:00 AM–12:00 PM

No on-call requirements – Dakota Home Care has dedicated on-call nursing staff who provide coverage outside regular business hours.

Starting salary: $80,000 annually

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
• Handle case assignments, draft care plans, review case progress and determine case closure
• Help clients achieve wellness and autonomy
• Facilitate multiple care aspects (case coordination, information sharing, etc)
• Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
• Develop effective working relations and cooperate with medical team throughout the entire case management process
• Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
• Record case information and accurately complete all necessary forms
• Promote quality and cost-effective interventions and outcomes
• Assess and address motivational and psychosocial goals
• Adhere to professional standards as outlined by agency protocols and state rules and regulations

Skills

• At least 3 years clinical experience as an RN
• Excellent organizational and time management skills
