Job summary

St. Tammany Health System is seeking a Clinical Documentation Specialist to enhance medical record documentation accuracy and representation of severity of illness, risk of mortality, and case complexity.

The CDS will participate in pre-visit and retrospective reviews to support the facility's risk adjustment strategy and ensure compliance with CMS guidelines. Ideal candidate is a registered nurse with strong knowledge of ICD-10 coding, risk adjustment, and medical necessity.

Qualifications

• Graduate of an accredited School of Nursing and current Louisiana RN license required.
• Knowledge of ICD-10 coding principles and guidelines and Risk Adjustment Coding.
• Proficiency with Microsoft Office (PowerPoint, Excel, Word), 3M software, and Encoder experience.

Responsibilities

• Facilitates the improved integrity of medical record documentation.
• Supports risk adjustment strategy through pre-visit and retrospective reviews.
• Educates and collaborates with physicians and APPs on documentation guidelines and coding requirements.
