Job summary

Continuum Hospice is seeking an experienced Registered Nurse to serve as Clinical Director in Petaluma, CA. You will lead an interdisciplinary team of RNs, aides, social workers, and chaplains, ensuring high-quality end-of-life care in line with regulations.

Responsibilities include coordinating patient services, guiding staff, conducting chart audits, and developing training programs. A BSN is preferred and a valid RN license is required; two years in hospice with supervisory experience is

Qualifications

• Current unencumbered RN license.
• Bachelor's degree in nursing from NLN-accredited program strongly preferred.
• Minimum 2 years as a professional nurse in hospice/home health in last 5 years with at least 1 year supervisory experience.
• Demonstrated ability to manage terminal care with crisis intervention, pain and symptom control, bedside nursing, and family counseling.
• Proficiency with computers/EMR and ability to type.
• CPR certification required.

• Coordinate and oversee a spectrum of patient services in line with hospice policies.
• Provide guidance and support to an interdisciplinary team of clinicians.
• Conduct chart audits to ensure regulatory and ethical standards.
• Develop orientation and in-service training for nursing/clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually, with coaching.
• Lead interdisciplinary team meetings to refine patient/family Care Plans.

Collaborating closely with your Program Director, Hospice Physician, and Community Liaisons, you'll orchestrate office excellence, foster growth in patient numbers, and guarantee adherence to Federal, State, ACHC, and Continuum Hospiceguidelines. Our RN Clinical Directors are strong leaders with a relentless drive to succeed, an unshakeable faith in their team, and remarkable emotional IQ.

Schedule: M-F 8a-4:30pm + after hour on call shifts

Responsibilities

• Champion Comprehensive Patient Support: Coordinate and oversee a spectrum of patient services, ensuring alignment with established hospice program policies.
• Empowering Teams: Provide guidance and support to an amazing team of clinicians, fostering continuous enhancement of hospice care services provided.
• Quality Assurance: Conduct meticulous chart audits, overseeing medical record upkeep for compliance with regulations, certification standards, and ethical care benchmarks.
• Training Excellence: Participate in crafting orientation and in-service training for nursing & clinical staff, nurturing their growth across all proficiency levels.
• Performance Advancement: Evaluate and uplift clinical staff performance during probation and annually, offering valuable teaching and counseling to enhance the delivery of services from our clinicians.
• Collaborative Leadership: Lead interdisciplinary team gatherings to refine patient/family Care Plans, encouraging the exchange of insights, resolution of challenges, and nurturing professional camaraderie.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing strongly preferred.
• A minimum of two (2) years experience as a professional nurse providing direct patient care in hospice, home health or communityhealth, in the last five (5) years, and a minimumone year of supervisory experience.
• Demonstrated ability to manage all aspects of terminal care with expertise in crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
• Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family needs.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$150,000 - $166,000 per year (Average Salary Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
