OhioHealth is seeking a program developer to lead care coordination and outreach for designated programs. The role integrates clinical management with multidisciplinary collaboration to improve patient outcomes.
Responsibilities include coordinating with physicians, EMS, and system service lines, while ensuring regulatory compliance and ongoing education. This position requires a Bachelor's degree and Ohio RN licensure, with the potential for clinical involvement as needed.

This position is responsible for the programmatic development of the assigned program(s). Programmatic development consists of care coordination and outreach management. Care coordination responsibilities include, but are not limited to, the clinical management of specialized multidisciplinary care pathways. Outreach management responsibilities include, but are not limited to, organized communication between care continuum stakeholders (e. g physician specialists, primary and consulting physicians, other healthcare facilities, EMS, and System Service Lines). Other duties include research, education, compliance with regulatory requirements, and the implementation of annual strategic objectives.
