Job summary

Amity Foundation is seeking an RN Clinical Consultant to support its Teaching and Therapeutic Community programs in California. This role provides nursing expertise, coordinates care for high-need individuals, and collaborates with leaders, healthcare providers, and community partners to ensure high-quality clinical services.

The RN will guide care coordination, implement evidence-based nursing practices, and contribute to quality improvement across programs, while maintaining confidentiality

Qualifications

• Current and unrestricted RN license in California (CA) in good standing.
• Graduate of an accredited nursing program.
• Minimum of 5 years of professional nursing/clinical experience.
• Experience with care management, population health, community health, behavioral health, or related clinical setting.
• Valid and unexpired Driver's License.
• Ability to meet background, clearance, and facility requirements.

Responsibilities

• Provide nursing consultation and clinical guidance to program faculty regarding student healthcare needs.
• Review and coordinate complex cases involving medical, behavioral health, and social service needs.
• Develop and implement clinical strategies aligning with program goals and objectives.
• Guide care coordination, follow-up care, and connections to healthcare providers and community resources.
• Review clinical information and identify next steps for students with complex or chronic conditions.
• Integrate evidence-based clinical and nursing practices into program services.
• Collaborate with leadership and care management on quality improvement initiatives.
• Serve as clinical liaison with healthcare providers, hospitals, and community resources.
• Participate in multidisciplinary meetings and case conferences for students with complex healthcare needs.
• Provide training to faculty on health topics, care coordination, and chronic condition management within RN scope.
• Provide clinical guidance to Advocates and faculty when needed.
• Identify health and safety concerns and recommend follow-up actions.
• Support continuity of care for individuals transitioning from incarceration, treatment, or hospitalization.

About the Position

The RN Clinical Consultant provides clinical nursing expertise, consultation, and guidance to support the delivery of high-quality services within Amity Foundation programs.

This position works collaboratively with program leadership, Advocates, care management teams, healthcare providers, and community partners to help address the medical and healthcare needs of individuals served. The RN Clinical Consultant assists with complex cases, care coordination, clinical documentation, health-related education, and the development of practices that support positive outcomes.

The ideal candidate will have a strong nursing background, experience working with individuals with complex medical and behavioral health needs, and an understanding of care coordination and community-based services.

What You Will Do

• Provide nursing consultation and clinical guidance to program faculty regarding student healthcare needs.

• Assist with the review and coordination of complex cases involving medical, behavioral health, and social service needs.

• Support the development and implementation of clinical strategies that align with program goals and objectives.

• Provide guidance regarding appropriate care coordination, follow-up care, and connections to healthcare providers and community resources.

• Review clinical information and assist faculty in identifying appropriate next steps for students with complex or chronic medical conditions.

• Support the integration of evidence-based clinical and nursing practices into program services.

• Assist with ensuring clinical documentation is accurate, current, and consistent with applicable program requirements and standards.

• Collaborate with program leadership and care management teams on quality improvement initiatives.

• Serve as a clinical liaison with healthcare providers, hospitals, health plans, and community-based resources as appropriate.

• Participate in multidisciplinary meetings and case consultations involving students with complex healthcare needs.

• Provide training and education to faculty on health-related topics, care coordination, chronic condition management, and other clinical areas within the RN scope of practice.

• Provide consultation and support to Advocates and other faculty when additional clinical guidance is needed.

• Assist with identifying potential health and safety concerns and provide recommendations for appropriate follow-up.

• Support continuity of care for individuals transitioning from incarceration, treatment, hospitalization, or other care settings into the community.

• Maintain appropriate professional and clinical boundaries and follow all applicable confidentiality requirements.

• Support the learning and practice of Amity Foundation's Teaching and Therapeutic Community model.

• Attend workshops, meetings, trainings, and case conferences as requested by the supervisor.

• Travel locally to program locations, healthcare facilities, meetings, and other locations as needed.

• Perform other related duties as assigned.

What You Will Bring

• Strong knowledge of professional nursing practices, clinical assessment, care coordination, and patient-centered care.

• Knowledge of chronic medical conditions and healthcare needs commonly experienced by high-need and underserved populations.

• Ability to review clinical information and provide appropriate nursing consultation and recommendations.

• Strong communication and interpersonal skills with the ability to collaborate with multidisciplinary teams.

• Ability to work effectively with healthcare providers, community partners, program leadership, and faculty.

• Strong organizational, documentation, and case-management skills.

• Ability to maintain confidentiality and appropriately handle sensitive medical and personal information.

• Ability to work with individuals from diverse backgrounds, including individuals impacted by incarceration, homelessness, substance use, trauma, and other barriers to care.

• Willingness to travel locally as needed to support program initiatives, trainings, and student care.

Required

• Current and unrestricted Registered Nurse (RN) license in the State of California, in good standing.

• Graduate of an accredited nursing program.

• Minimum of 5 years of professional nursing/clinical experience.

• Experience with care management, case management, population health, community health, behavioral health, or a related clinical setting.

• Valid and unexpired Driver's License.

• Ability to successfully meet applicable background, clearance, and facility requirements.

Preferred

• Bachelor's degree in Nursing (BSN) preferred.

• Experience working with individuals recently released from jail or prison.

• Experience working with individuals with substance use, behavioral health, chronic medical, or complex care needs.

• Previous experience in Enhanced Care Management (ECM), Community Supports, managed care, or similar programs.

• Previous supervisory, clinical leadership, or staff-training experience.
