Job summary

A healthcare staffing agency in the New York area is seeking an experienced Registered Nurse (Clinical Care Manager) to lead care coordination and clinical quality in a community-based setting. The ideal candidate should have over 2 years of clinical management experience, a valid RN license in New York, and strong EMR and UAS assessment skills. Competitive pay of $57-$61 per hour with a Monday to Friday schedule is offered.

Qualifications

• 2+ years in a clinical management role with elderly populations.
• Active and unrestricted Registered Nurse license in New York State required.
• Completion of NYSED-approved infection control coursework required.

Responsibilities

• Manage individualized care plans and facilitate care transitions.
• Supervise nursing staff and oversee scheduling and performance.
• Lead interdisciplinary team meetings to align care with participant needs.
• Ensure complete, accurate records while maintaining regulatory compliance.
• Monitor clinical documentation and improve safety and care standards.
