VitalCaring seeks a Clinical Care Manager (RN) for its Vinita, OK Home Health branch. You will lead coordination of care, guide clinical decisions, and ensure high-quality patient outcomes across the full care journey.

Ideal candidates are RNs with state licensure, strong leadership, and experience in home health or care coordination. This is a full-time, on-site role offering comprehensive benefits and a supportive, growth-oriented culture.

Qualifications

• Active RN license in state of practice.
• BSN preferred.
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with ability to collaborate across disciplines.
• Comfortable working independently in a fast-paced, field-based environment.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
