Responsibilities

  • Initiate, develop, implement, and revise the plan of care in collaboration with physicians and healthcare professionals involved in care.
  • Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction, and assign tasks according to state and federal regulations. Conduct supervisory visits as required.
  • Provide high-quality clinical services within your scope of practice and infection control standards, in accordance with the plan of care, and in coordination with the healthcare team.
  • Complete comprehensive assessments (OASIS) accurately and timely, including medication reconciliation.
  • Document patient visits thoroughly and accurately, adhering to policy and payer requirements, and ensure timely submission per LHC policy.

Education and Experience

License Requirements:

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Valid Driver's License, vehicle insurance, and access to a dependable vehicle
