Job summary

iCare Health Network in Vermont seeks a Charge Nurse to provide medication administration, treatments, and assist residents with activities of daily living in our skilled nursing facility.

You will coordinate and supervise staff, document patient changes, and collaborate with the RN Supervisor. A VT nursing license and at least 1 year in acute or long-term care are required; a sign-on bonus up to $5,000 is offered.

Qualifications

• Current VT nursing license in good standing.
• Minimum of 1 year of experience in acute or long-term care in a skilled nursing facility.
• Experience in rehabilitation or behavioral health is a plus.
• Proficiency in English (speaking, reading, and writing).
• EMR experience preferred.

Responsibilities

• Plan and implement the highest possible quality of care for each resident on the assigned unit.
• Coordinate, assign, direct and prioritize work of staff, mainly LNAs, to ensure efficient operation.
• Make independent nursing decisions and collaborate with the RN Supervisor as needed.
• Document changes in patient condition, new orders, and incidents; report to the RN Supervisor.

Summary:

Primary responsibilities of the Charge Nurse include providing medication administration, treatments, and assistance with activities of daily living for facility residents.

Responsibilities:

• Plans and implements the optimal achievable quality of care rendered to each resident on the assigned Unit

• The Charge Nurse shall coordinate, assign, direct and prioritize the work of other staff members (primarily LNAs) in order to ensure the efficient operation of the nursing unit. .

• Makes independent decisions relevant to nursing care which are required and delivered to each resident as per the nursing assignments. Collaborates with RN Supervisor as relevant decisions are required, in a timely manner.

• Documents change in patient’s condition, new orders received, unusual occurrences and report of incidents including employee injuries. Reports unusual occurrences to the RN Supervisor.

Qualifications:

• A current, unencumbered VT nursing license (Reimbursement for out-of-state license transfer)

• Prefer a minimum of one year of experience in acute care and/or long term care in a skilled nursing facility

• Experience in rehabilitation or behavioral health is a plus

• Proficiency in English, including speaking, reading, and writing, required

• EMR experience preferred
