Job summary

Interim HealthCare Inc. in Bridgeport, West Virginia is seeking an experienced Registered Nurse Case Manager (RNCM) to deliver compassionate, in-home care.

You will assess patients, develop and monitor care plans, and lead care teams including aides, LPNs, and other nurses to ensure continuity and quality of service. Responsibilities include conducting in-person interviews, updating plans of care, educating patients and families, coordinating with providers, and working to reduce readmissions

Qualifications

• Active RN license in the state for this position.
• CPR/AED/BLS/First Aid certification required.
• Minimum 1 year RN experience in home health or similar setting; case management/utilization review preferred.
• OASIS experience preferred.
• Practical trach/ventilator experience preferred, not required.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee implementation and ongoing assessment of the care plan with home health aides, LPNs, RNs, and other caregivers.
• Communicate patient conditions and collaborate with providers to adjust care as needs evolve.
• Educate patients and families on home health care procedures (wound care, IV administration, medication management).
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.

To qualify as a Registered Nurse Case Manager (RNCM) with us, you will need:

• Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certificationReliable transportation to/from care sites and/or work locations.
• One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management/utilization review experience preferred.
• OASIS experience preferred.
• Practical trach and/or ventilator experience preferred, not required.
