Job summary

UCLA Health is seeking an experienced nurse case manager for a fully on-site role in Los Angeles. You will assess and coordinate care for diverse patients, collaborating with a multidisciplinary team to ensure safe and efficient care.

Expect 4-day, 10-hour shifts (noon–10:30 pm) with potential weekends and holidays. Responsibilities include developing individualized care plans, conducting risk stratification, and performing utilization review while driving performance improvement.

Qualifications

  • BSN or MSN (ASN accepted for current UCLA Health Nursing staff).
  • CA RN License and BLS certification.
  • Recent experience in case management, utilization management and discharge planning.
  • Minimum of three years of acute hospital experience; or the equivalent of education and experience.
  • Strong leadership abilities and ability to work with interdisciplinary teams.

Responsibilities

  • Assess and coordinate care for a diverse group of patients with advanced practice nursing skills.
  • Collaborate with a multi-disciplinary health care team and patients’ families to ensure safe, effective care.
  • Develop and implement individualized care plans using evidence-based tools for risk stratification.
  • Perform utilization review and contribute to performance improvement initiatives.

Using your advanced practice nursing skills, you will be responsible for assessing and coordinating care for a diverse group of patients. You will collaborate and consult with a multi-disciplinary health care team as well as with patients and their families to ensure safe and effective coordination of care. This involves developing and implementing individualized care plans utilizing evidence-based tools for risk stratification to ensure delivery of safe, high quality, efficient, and cost-effective care. You will also perform utilization review while assuring the delivery of concurrent and post-hospital care. We’re also looking to you to help drive performance improvement efforts.

Salary Range: $68.81 - 88.99 Hourly

Job Qualifications

We’re seeking a self-directed, creative problem solver with a:

  • BSN or MSN (ASN accepted for current UCLA Health Nursing staff)
  • CA RN License and BLS certification
  • Recent experience in case management, utilization management and discharge planning
  • Minimum of three years of acute hospital experience; or the equivalent of education and experience
  • Strong leadership abilities
  • Systems planning and patient care management experience in a high-volume work environment
  • Excellent communication, interpersonal, organizational and analytical skills
  • Ability to work effectively and collaboratively with interdisciplinary teams
  • Knowledge of a large university teaching hospitals

Preferred: certified case manager and acute hospital experience as a case manager.
