Job summary

You will arrange and document post-acute care services, collaborate with a multidisciplinary team, and support smooth transitions of care. Requires ASN, FL RN license, BLS; BSN preferred; 1 year case management experience.

Qualifications

• 1 year case management experience.
• ASN required; BSN preferred.
• FL Licensed Registered Nurse required; BLS certification required.
