Job summary

Job description

This position handles multidisciplinary communications and facilitation of care throughout a patient's hospitalization, including review of resource utilization and ensuring medically necessary care criteria. The role involves acting as a liaison among patients, caregivers, multidisciplinary team members, post‑acute and third‑party payers, and supporting the development and implementation of individualized discharge plans.

Education
  • Graduate of an accredited school of nursing (Required).
  • Bachelor’s degree in nursing (Preferred).
Work Experience
  • None (Required).
  • 3 years of hospital‑based experience in discharge planning, case management, or utilization review (Preferred).
Certifications
  • Current registered nurse license in state of practice (Required).
  • Basic Life Support (BLS) from the American Heart Association (Required).
  • Certification in Case Management (CCM) or Accredited Case Manager (ACM) (Preferred).
Knowledge, Skills and Abilities
  • Proficiency in using computers, software, and web‑based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Strong interpersonal skills.
  • Ability to work a flexible work schedule (e.g., 24/7, weekend, holiday, on‑call availability).
Job Duties
  • Perform pre‑admission, admission, and continued stay reviews as directed per CMS guidelines, commercial payer guidelines, and/or hospital policy.
  • Support the values of service excellence, teamwork, and integrity and maintain minimum standards for licensure while striving for personal/professional development.
  • Maintain knowledge of the conditions of participation as it relates to utilization review and discharge planning.
  • Maintain open communication with all appropriate parties while functioning as a liaison between ED physicians, hospitalists, attending physicians, and physician advisors.
  • Lead the oversight and management of the functional and financial outcomes, including coordination and implementation of the discharge plan, during the acute‑care hospitalization.
  • Adapt behavior to the specific patient population, including respect for privacy, appropriate introduction and explanation of services, requesting permissions, and communication style.
  • Perform other related duties as required.
Physical and Environmental Demands

Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Duties involve walking or standing substantially; may require pushing or pulling. The role involves routine exposure to blood, body fluid and tissue, and potential exposure to hazardous medications or waste. The risk level of exposure may increase depending on essential job duties.
