HCA HealthONE Swedish in Colorado is actively seeking a Registered Nurse to coordinate interdisciplinary care, manage admissions and discharge planning, and ensure quality and safety throughout a patient’s hospital stay. You will work within a team to balance patient needs with efficient resource use.

Ideal candidates hold a Colorado RN license (or compact multistate), BLS certification, and 3–5 years in clinical or case management roles.

Qualifications

• Current Colorado RN license or active multistate license.
• BLS certification through AHA or ARC.
• BSN preferred.
• 3–5 years of recent clinical or case management experience.
• Strong communication and analytical skills.

Responsabilités

• Coordinate services through an interdisciplinary process balancing patient and family needs with resource use.
• Conduct concurrent case management to assess admission, hospitalization level of care, and discharge planning.
• Monitor quality and risk management and report to the QRM department as part of the CQI process.

Job Summary And Qualifications

Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues will also be monitored for and reported to the QRM department as a part of the CQI process.

II. Position Requirements:

• Licensure/Certification/Registration:

Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure. BLS certification through AHA or ARC.

• Education:

Graduate of an accredited school of nursing; BSN preferred.

• Experience:

3-5 years recent clinical or case management experience in area of specialization.

• Knowledge/Skills/Abilities:

Requires excellent human relations and oral/written communication skills. Must have the ability to asses’ information, apply critical thinking processes to determine course of action, define expected outcomes, and evaluate effectiveness of plan through continuous assessment of the patient's progress. Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Must also have strong analytical and organizational skills.
