Job summary

Ascend Hospice is hiring an RN Case Manager on a per diem basis to support on-call and case management activities. You will assess patients and families, develop care plans, and collaborate with the care team to ensure continuous, well-coordinated hospice services.

Travel is required to patient sites and visits are part of the role. The ideal candidate holds a State Certified RN license, has hospice experience (preferred), and demonstrates strong communication skills.

Qualifications

• Must have strong verbal and written communication skills.
• Must have a State Certified RN license.
• Must be willing to travel and have reliable transportation.
• 2 years' experience as a professional nurse, hospice experience preferred.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family to direct work for continuous, well-coordinated care.
• Counsel the patient and family in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes in condition.
• Supervise the Hospice Aides.

Hours:

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:
