This full-time position offers four 10-hour shifts (Tue–Fri) with a pay range of $53–$79 per hour. Licensure in OR (or WA Compact) and at least five years of nursing, including two years in case management, are required.

Qualifications

• Minimum five years of nursing experience in acute or ambulatory care, with two years in case management.
• One year of ambulatory oncology experience within the past 3 years.
• One year of telephone triage within the past 3 years.

Responsibilities

• Identify members for enrollment into case management using regional population data.
• Complete chart reviews, screening calls, and full assessments for care level.
• Triage findings and engage patients to define a plan of care.
• Develop and document a patient-centered care plan with measurable goals.

What You'll Do

• Utilize regional population stratification information to identify members for enrollment into case management.
• Independently complete chart reviews, screening calls, and full assessments related to the anticipated level of care.
• Triage findings from member assessments and engage patients to define a plan of care.
• Develop and document a patient-centered care plan with specific, attainable, and measurable goals.
• Provide care coordination and management services for members with identified needs.
• Strengthen and improve Case Management Services by establishing strong relationships with clinicians.
• Act as a liaison between the patient and appropriate care delivery team.

Requirements

Education

• Graduate of accredited school of nursing.

Experience

• Minimum five (5) years of nursing experience in acute care or ambulatory care, including two (2) years in a case management/care coordination role.
• 1 year of ambulatory oncology experience within the past 3 years.
• 1 year of telephone triage within the past 3 years.

Licenses & Certifications

• Registered Nurse License (Washington) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire.
• Registered Nurse License (Oregon) within 6 months of hire.
• Basic Life Support required at hire.
