Job summary

Notre Dame Hospice in New Orleans is seeking an experienced RN Case Manager to serve the New Orleans and Northshore area. You will coordinate patient care, document progress, and collaborate with the IDT to ensure high-quality hospice services.

Starting pay is based on experience; travel to patient homes is required. This full-time role requires a Louisiana RN license and a compassionate, professional approach.

Qualifications

Responsibilities

• Identify patient/family needs and reassess as needed.
• Provide nursing services per the POC.

• Instruct patient/family in self-care when appropriate.
• Supervise ancillary personnel and delegate as required.
• Submit accurate clinical notes and perform supervisory visits.

Essential Qualifications:

Responsibilities

• Identify the patient/family's physical, psychosocial, and environmental needs and reassess as needed.
• Provide nursing services in accordance with the POC.
• Document problems, appropriate goals, interventions, and patient/family response to hospice care.
• Collaborate with the patient/family attending physician and other members of the IDT in providing patient and family care.
• Instruct patient/family in self-care techniques when appropriate.
• Supervise ancillary personnel and delegates responsibilities when required.
• Complete and submit accurate and relevant clinical notes regarding the patient's condition as required.
• Perform supervisory visits to the patient's residence.

Qualifications:

• Applicants should possess a current Louisiana RN license in good standing and have at least one year of nursing experience. Home health experience is preferred, but we will train eager candidates with good clinical experience.

Other Qualifications

• Has not been excluded from participating in Medicare and Medicaid programs.
• Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
• Capable of providing emergency care as needed for any patient.
• Maintains a neat, well-groomed appearance at all times / Keeps personal appearance appropriate for patient care.
