Job summary

Decypher is seeking a dedicated and experienced Registered Nurse Case Manager (RN, CCM) to support the health and readiness of America’s service members, veterans, and their families. This full-time role partners with the Defense Health Agency to deliver high-quality discharge planning and comprehensive case management in a mission-driven healthcare setting.

You will collaborate with interdisciplinary teams, advocate for patient-centered outcomes, and ensure accurate documentation and regulatory

Qualifications

• Associate Degree in Nursing required; BSN preferred.
• Current, full, active RN license.
• BLS certification required.
• At least one of the listed advanced nursing/case-management certifications.
• For ADN holders: four years discharge planning or case management experience.
• For BSN holders: two years discharge planning or case management experience in last 36 months.
• U.S. Citizenship is required.

Responsibilities

• Participate in all phases of CMP to ensure standards and protocols.
• Provide expert nursing input in assessment, planning, coordination, and evaluation.
• Identify and integrate local case management processes for improvement.
• Develop local strategies using inpatient, outpatient, onsite, and telephonic methods.
• Integrate case management with utilization management and social work.
• Collaborate with interdisciplinary teams to facilitate transitions and discharge planning.
• Maintain detailed documentation in compliance with healthcare standards.
