Job summary

Heart to Heart Hospice is seeking an RN Case Manager to join our compassionate team. The role involves comprehensive assessments of patients with life-limiting illnesses, developing and updating care plans, and coordinating care with physicians and the IDG.

On-call duties and thorough documentation are required. Eligible candidates will have a Nursing degree (Diploma/Associate/BSN) and an active RN license, plus a valid driver's license and insured vehicle.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and valid auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborates with physicians and healthcare professionals across the care team.
• Attend IDG meetings and coordinate patient care planning.
• Provide on-call service to patients and families as assigned.
• Ensure timely documentation and compliance with laws and policies.

RN Case Manager

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
