Job summary

HCA Florida Gulf Coast Hospital is seeking a dynamic Case Management RN to join our inpatient rehab team. You will coordinate interdisciplinary care, evaluate medical necessity, and support patient throughput while ensuring quality outcomes.

Applicants must hold Florida RN license, BLS, and 3 years RN acute care experience; BSN and Case Management certification preferred. This PRN role offers mentorship and growth within a respected network.

Qualifications

• Current Florida RN license with ability to obtain multistate license if applicable.
• BLS certification required within 30 days of start.
• Minimum 3 years of RN experience in an acute care setting.
• Case Management experience preferred; certification preferred.

Responsibilities

• Performs comprehensive psychosocial and medical assessments.
• Advocates for patients and leads the interdisciplinary team.
• Coordinates plan of care and manages utilization reviews.
• Liaises with physicians, patients, families, and outside agencies.
• Supports patient throughput with focus on quality and efficiency.

Job Summary and Qualifications

We areseeking a dynamicCase ManagementRN to join ourteam.We need a compassionate and reliable RN tofacilitatethe interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. In thisroleyou willbe requiredtocoordinate activities that promote quality outcomes and patient throughput while supporting a balance ofoptimalcare andappropriate resourceutilization. WHAT YOU WILL DO IN THIS ROLE:

• Performs a comprehensive assessment of psychosocial and medical needs of patients
• Advocatesfor patients while assuming a leadership role withthe interdisciplinary team
• Coordinates plan of care
• Acts as a liaison through effective and professional communications with physicians, patients, families, hospital staff, and outside agencies
• Performsutilizationmanagement reviews and communicates information to third party payors
• Facilitates patient throughput with an ongoing focus on quality and efficiency

Requirements:

• Florida RN license and/or approved multistate RN licenserequired. Compact license holders have60 daysto obtain the Florida license after relocation.
• BLS-AHA Provider issued cardrequiredwithin30 daysof start
• 3yearsof RN experience inan acute care setting isrequired
• Prior experience in Case Management is preferred
• BSN is preferred
• Certification in Case Management, Nursing or Utilization Review ispreferred
