Cambridge Health Alliance (CHA) is seeking a Per Diem On Call RN Case Manager at CHA Cambridge Hospital. Under supervision of the Director of Care Management, this role provides clinically-based case management for adults with psychiatric, medical, or substance use needs.

The RN collaborates with the care team to optimize resources, ensure reimbursement, and plan post-acute care with patient and family. The position requires a BSN (preferred), MA RN licensure, and ARC/AHA BLS certification.

Qualifications

  • BSN preferred.
  • Current MA RN Licensure.
  • Case Management certification preferred.
  • ARC or AHA BLS certification required.
  • Hospital Care Management Experience strongly preferred.
  • 3-5 years of medical/surgical RN experience OR 3-5 years of RN Case Management experience required.

Responsibilities

  • Provide clinically-based case management to support effective patient care.
  • Collaborate with health care team to determine resource utilization and reimbursement.
  • Determine medical necessity for admission and continued stay using criteria.
  • Develop post-acute care plans with patient and families for transitions.
  • Cross-train to care management areas as needed.

Department Description

Inpatient Case Management provides psychosocial assessments, evaluations, and referrals for adults, and/or families with psychiatric illness, substance abuse, and medical illness. Casework or therapy takes place in the hospital setting. Cross Training to all areas of Care Management

Summary

Under the general supervision of the Director of Care Management, the RN Case Manager provides clinically-based case management to support the delivery of effective and efficient patient care consistent with the Centers for Medicaid and Medicare Conditions of Participation. The RN Case Manager will collaborate with other members of the health care team to identify appropriate utilization of resources and to ensure reimbursement. Utilize criteria to confirm medical necessity for admission and continued stay. With the patient, family and health care team, create a post-acute care plan appropriate to the patient's needs and resources and facilitate this transition in concert with that patient's inpatient and post-acute care teams.

Qualifications/Experience
  • Education: Bachelor of Science in Nursing (BSN) degree preferred.
  • Current or Conditional MA RN Licensure
  • Certification in Case Management preferred.
  • American Red Cross (ARC) or American Heart Association (AHA)BLS certification required
  • Experience: Hospital Care Management Experience strongly preferred.
  • 3-5 years of medical/surgical RN experience OR 3-5 years of other RN Case Management experience is required.
