Job summary

Serenity Hospice is seeking an RN Case Manager to deliver compassionate hospice care to patients and families across New Jersey. You will create individualized care plans, perform regular assessments, and coordinate with families and the healthcare team to ensure seamless, patient-centered end-of-life support.

The role covers Union, Essex, and Hudson counties, requires a current NJ RN license, and travel to assigned areas.

Qualifications

• Current NJ RN License in good standing.
• Hospice experience preferred.
• Willing to travel to assigned areas.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family to direct care that is continuous and well-coordinated.
• RN counsels the patient and family in meeting end of life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician regarding patient's needs and changes.
• Supervise Hospice Aides and LPNs.
