Job summary

Friendship Village STL is seeking a qualified RN Case Manager to provide compassionate end-of-life care and support for residents in our premier Life Plan Community in St. Louis. This role emphasizes quality of life, teamwork, and family-centered care within a collaborative care team.

You will coordinate care plans, monitor patient status, and work with interdisciplinary staff to ensure physical, emotional, and spiritual needs are met. A Missouri RN license and strong communication are essential.

Qualifications

• Active RN license in Missouri.
• Experience in end-of-life or palliative care preferred.
• Strong patient- and family-centered communication.

The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The RN Case Manager is typically part of a care team. This team collaborates to ensure individuals and their families have the physical, emotional, and spiritual support they require throughout the care process.
