Job summary

Amedisys Home Health in Uniontown and surrounding areas is hiring a Registered Nurse case manager for full-time days. You will assess patients, collaborate with the care team and implement care plans to help patients heal at home.

Responsibilities include referrals, documenting progress, teaching families, and coordinating care, with on-call duties as assigned. Experience as an RN and CPR are required, with a valid state license and driver’s license.

Qualifications

• One year of nursing experience.
• Current RN license, state-specific where you work.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

$88,000.00-$92,000.00 Annual Salary Range
