AMN Healthcare is seeking a Hospice RN Case Manager for the East Bay Area. You will admit patients, assess needs, develop care plans, and coordinate with a multidisciplinary team to ensure quality patient care.

The role requires CA RN license, BLS, and at least 1 year of hospice case management experience. The assignment runs for 13 weeks with a schedule of 8:30am-5pm and one weekend per month.

Competenze

• CA RN license required.
• BLS certification required.
• 1 year recent hospice case management experience.

Mansioni

• Admits patients to service and assesses the patient’s and family’s needs (physical, psychosocial, environmental, safety and developmental).
• Develops individualized care plans based on assessment and documents observations and interventions.
• Coordinates care with a multidisciplinary team and participates in QA/QI programs; conducts follow-up visits as needed.

POSITION SUMMARY – RN Case Manager - Hospice

Admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed. Territory is the East Bay Area.

Minimum Required Qualifications –

• CA RN license
• BLS
• 1 year recent hospice case management experience

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

8:30am-5pm (Schedule can be flexible based on patient's availability.) Weekends: One weekend per month.
