Moments Hospice seeks an RN Case Manager to lead clinical care for a patient cohort, coordinating symptoms, care plans, and family support. You will be the primary nurse liaison, guiding care teams and ensuring comfort at end of life.

The role requires an active RN license, EMR proficiency, and strong organizational skills. A valid driver's license and reliable vehicle are needed; a fleet car program is offered.

Qualifications

  • Strong organizational, communication, and critical-thinking skills.
  • Ability to work independently while maintaining high standards of care.

Responsibilities

  • Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
  • Develop, implement, and update individualized plan of care in partnership with patients and families.
  • Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
  • Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
  • Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
  • Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
