Job summary

Suite Living Hospice Care in Minnesota is seeking an RN Case Manager to join our hospice team. You will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.

The role offers a Monday–Friday schedule, a localized territory in the northwest metro, and opportunities to build strong relationships with patients and families, with on-call rotation and competitive benefits.

Qualifications

• Active RN license in Minnesota.
• Graduate of an accredited School of Nursing.
• CPR certification.
• 1+ year of acute care experience.
• Hospice or home care experience preferred.
• Valid driver's license and insured vehicle.
• Strong assessment and clinical decision-making skills.

Responsibilities

• Conduct initial and ongoing patient assessments.
• Develop and manage individualized hospice care plans.
• Administer medications and provide palliative interventions.
• Educate patients and families on symptom management and care needs.
• Provide emotional support to patients and families.
• Manage and coordinate care for assigned patients.
• Collaborate with physicians and interdisciplinary team members.
• Participate in care conferences and family meetings.
• Advocate for patient needs and ensure access to resources.
• Maintain accurate and timely clinical documentation.
• Supervise and support hospice aides.
• Ensure necessary equipment and supplies are in place.
• Participate in on-call rotation (5 days/month).
• Contribute to quality improvement initiatives.

Position Overview

What You'll Do

Patient Care & Case Management
