Job summary

Amedisys is seeking an RN case manager for full-time day shifts in Lancaster and Lebanon Counties, PA. You will assess patients, plan and coordinate home-based care, and collaborate with the hospice team to optimize comfort and outcomes.

Eligible candidates bring 1 year nursing experience, an active RN license, CPR certification, and a valid driver’s license. This role offers a substantial sign-on bonus and comprehensive benefits, including healthcare plans, time off, and retirement options.

Qualifications

• One year of nursing experience.
• Current RN license, state-specific where assigned.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.
