Job summary

Amedisys is seeking an RN Case Manager to coordinate patient care in the home setting. You will assess patients, develop care plans with the team, and refer to hospice disciplines as needed while documenting progress and outcomes.

The role requires an active RN license in relevant states, CPR certification, and a valid driver’s license with reliable transportation. Join a trusted home health leader and make a meaningful impact at patients’ homes.

Qualifications

• One year of nursing experience.
• Current RN license, state(s) you work in.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records accurately and timely.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates patients, families, and caregivers on care techniques.
• Regularly communicates with the clinical manager and care team.
• Performs on-call responsibilities to patients/families as assigned.
• Other duties as assigned.
