Job summary

Amedisys Home Health is seeking an RN Case Manager to join our team in Greeneville, Tennessee. You will assess patients at home, collaborate with the care team to implement a personalized plan of care, and coordinate referrals as needed.

You will maintain accurate progress records, monitor pain and symptom changes, educate patients and families, and participate in on-call coverage as required. A valid RN license and CPR certification are required.

Qualifications

• Nursing experience of at least 1 year.
• Current RN license valid for the states you work.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
