Job summary

Amedisys, Inc. is seeking a registered nurse case manager to support patients at home within our home health and hospice services. You will assess patients, coordinate with the care team, and implement comprehensive plans of care to promote healing in the comfort of patients’ homes.

Responsibilities include developing individualized care records, on-call duties, and educating families on care techniques. A valid RN license and CPR are required, along with a driver's license and reliable

Qualifications

• One year of nursing experience.
• Current RN license, valid in the assigned state.
• CPR certification is required.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records according to center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
