Job summary

Mission Healthcare is seeking a registered nurse to provide high-quality home health care in Stanislaus, Merced, and San Joaquin Counties. You will perform initial assessments, develop and adjust care plans, and work closely with patients and families to meet their needs.

In this role, you’ll administer medications, provide education, and ensure smooth discharge planning. A current RN license, BLS CPR, and 1+ year of acute care experience are required, with a valid driver’s license and insurance.

Qualifications

Current and valid state RN license.
Minimum of 1 year of recent acute care experience in an institutional setting.
BLS/CPR certification is required.
Valid driver's license and auto insurance.
Associate's or Bachelor's degree in Nursing; home health experience preferred.

Responsibilities

Complete an initial patient and family assessment to determine home care needs and document history of illness.
Initiate and revise the plan of care as patient status changes.
Regularly re-evaluate patient needs and progress.
Use health data to determine nursing diagnoses.
Develop and implement a care plan with goals based on diagnosis and family involvement.
Administer medications and treatments as prescribed by the physician.
Counsel patient and family on the plan of care and needs.
Provide health care instructions and plan discharge planning.
