Job summary

Friendship Village in St. Louis is seeking a Registered Nurse Case Manager for its home health program. The role focuses on delivering professional nursing services per physician orders, assessing patient needs, and developing care plans to solve nursing problems for a defined group of residents.

You will coordinate care, initiate plans or delegate tasks, and evaluate outcomes while completing required policy training in a supportive team environment.

The RN Case Manager is responsible for determining the nursing problems for an assigned group of patients/residents, making plans to solve them, initiating the plan or assigning others to implement it, and evaluating the extent to which the plan was effective in resolving the problems identified.
