Nurturing Home Healthcare Agency Inc. is seeking an experienced RN Case Manager to oversee a home health caseload, coordinate care with physicians, therapists, and caregivers, and ensure compliance with Medicare rules and agency policies.

Responsabilidades

• Manage and oversee an assigned home health patient caseload.
• Review and approve clinician documentation for accuracy, completeness, and regulatory compliance.
• Coordinate patient care with physicians, field clinicians, patients, caregivers, and the interdisciplinary team.
• Communicate changes in patient condition to physicians and obtain necessary verbal and written orders.
• Monitor patient progress and coordinate timely follow-up care.
• Participate in interdisciplinary team meetings and case conferences.
• Maintain accurate patient records within Kinnser (WellSky) EMR.
• Support field clinicians with clinical guidance and problem-solving to ensure quality patient care.
• Ensure all documentation is completed within agency timelines and compliance standards.

Position Summary

The RN Case Manager is responsible for overseeing a caseload of home health patients, coordinating services between physicians, field clinicians, patients, and caregivers while ensuring compliance with Medicare regulations and agency policies. This position plays a critical role in delivering quality patient outcomes through effective communication, clinical oversight, and care coordination.
