Responsibilities:

• Facilitate Personalized Plans of Care: Work closely with patients and their families to develop and implement individualized care plans that address their unique needs and preferences.
• Perform Skilled Nursing Services: Provide expert nursing care, ensuring that clinical procedures and protocols are followed to maintain the highest standards of healthcare.
• Supervision: Oversee and guide a team of skilled healthcare professionals, ensuring the delivery of quality care that aligns with our commitment to excellence.
• Collaborate with Interdisciplinary Teams: Engage in effective communication and collaboration with therapists and other healthcare professionals to enhance the overall well-being of our residents.

Qualifications:

• Valid and current State of MN RN license.
• Proven experience in home care or a related healthcare setting.
• Strong organizational and interpersonal skills.
• Ability to develop and implement comprehensive care plans.
