Essential Functions

• Providehands-on,skilled nursing carein accordance withphysician’s orders.
• Providehealth educationto the patient and family.
• Assess patient response to the plan of care interventions and communicate changes to the case manager.
• Collaborate and share findings with the multidisciplinary care team.
• Document those servicesrenderedto the patient, notechangesin the patient’s condition and/or family and home situations.
• Document communication with case manager and physician.
• Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager.
• Other duties as assigned.

Education Requirements

• Current, validRN license issued byNYSDepartment of Education.

TheRN Procedure Nurseis primarily ina non-officesetting and may be exposed to outdoor conditions.

The working conditions are classified as medium work:
