UnitedHealth Group is seeking a Nurse Case Manager in Savannah, GA to guide patients and families through care needs, coordinate services, and support transition planning. You will work under clinical supervision to ensure appropriate levels of care and provide education and support for decisions.

Responsibilities include coordinating with the care team, referring to disciplines as needed, and delivering timely documentation while contributing to clinical development and ongoing education.

Competenze

• Current RN license, state(s) of assignment.
• 1+ year of clinical RN experience.
• CPR certification is required.
• Valid driver's license with reliable transportation.

As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker.

Primary Responsibilities:

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care


• Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so


• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques


• Supervises LPNs and HHAs


• Completes documentation timely and accurately


• Regularly communicate patient progress to the clinical manager and care team


• Plans and provides staff education


• Performs on-call responsibilities and on-call services to patients/families as assigned


• Participates in clinical development and continuing education programs


• Other duties as assigned

Required Qualifications:

• Current RN license, specific to the state(s) you are assigned to work


• 1+ year of clinical experience as a Registered Nurse (RN)


• Current CPR certification


• Valid driver's license, reliable transportation and liability insurance

Pay Range

• $71,677 - $107,516 annual total cash target pay


• $41.35 - $62.03 per visit point


• $32.46 - $51.69 hourly rate
