Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care needs.
• Leads discharge planning and coordinates patient transfers to other facilities.
• Performs chart reviews to address service delivery, outcomes, compliance, cost and reimbursement.
• Communicates with providers, payors, and ancillary services to ensure timely care progression.
• Documents case management activities, plans and outcomes and participates in performance improvement.
