Job summary

Healthier-Mississippi-People-LLC is seeking an RN-Case Manager I in Mississippi to support patients through coordinated discharge planning and care transitions. You will work within UMC nursing and case management practices to ensure right care at the right time and location, from admission through post-acute care handoffs.

Responsibilities include comprehensive patient assessments, multi-disciplinary plan development, and timely documentation.

Qualifications

• Must be licensed as RN and have at least one year acute care experience.
• Utilization management or previous case management experience preferred.

Responsibilities

• Coordinates a multi-professional plan of care addressing discharge needs.
• Comprehensive assessment of clinical discharge needs using nursing processes.
• Coordination of care, services, follow-up assessments, eligibility, and monitoring for discharge planning.
• Rounds with attending or resident staff to identify discharge plans and provide clinically-based recommendations.
• Maintain timely medical record documentation and communication with care team to ensure continuity of care and avoid delays.
• Organizes access to tests, procedures, and diagnostic results to manage length-of-stay.
• Collaborates with social workers and financial counselors to secure resources for patients and families.
