Qualifications

• Associates Degree required and AZ RN license.
• BLS certification (AHA) preferred or required.
• 3+ years related RN case management experience required.

Responsibilities

• Coordinate/facilitate patient care progression across the continuum with the care team.
• Collaborate with physicians and nursing staff to ensure timely, appropriate patient management.
• Complete utilization management and quality screening for assigned patients.
• Manage all aspects of discharge planning and ensure continuity of care.
• Document plan of care, communication with payers, and transitions of care.
• Participate in performance improvement activities and monitor key metrics.
