Heartlinks Hospice is seeking an RN Case Manager in Grandview, WA. The role involves planning, coordinating, and delivering in-home hospice care to patients and families, with emphasis on community health education.

You will assess needs, develop and revise plans of care, and collaborate with the clinical team to ensure high-quality patient outcomes. The position requires Washington RN licensure, BLS, and a valid driver’s license.

Qualifications

  • RN with current licensure to practice in Washington.
  • Graduate of an accredited school of nursing.
  • Maintains a current BLS certification.
  • Must have a valid driver's license with insured vehicle.
  • Ability to participate in backup on-call rotation.
  • Two years' experience in public health, home care, or hospice nursing preferred.

Responsibilities

  • Completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illnesses.
  • Provides professional nursing care using the nursing process.
  • Writes and initiates a plan of care; re-evaluates patient and family needs as necessary.
  • Coordinates patient care for an average caseload of 12–15 patients.
  • Assists patients with activities of daily living and supports self-sufficiency and comfort care.
  • Acts as Case Manager when assigned by Clinical Manager.

Job Description Summary:

The RN Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The registered nurse visits patients in their own home, whether that is a personal residence, assisted living, skilled nursing, adult family home, etc.

  • Completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by:
  • Writing and initiating a plan of care
  • Regularly re-evaluating patient and family/caregiver needs
  • Participating in revising the plan of care as necessary
  • Uses health assessment data to determine nursing diagnosis
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family to meet nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
  • Acts as Case Manager when assigned by Clinical Manager and coordinates patient care for assigned on average 12 - 15 patient caseload.

Position Qualifications:

  • Registered nurse with current licensure to practice professional nursing in the state of Washington.
  • Graduate of an accredited school of nursing.
  • Maintains a current BLS certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Ability to participate in the backup on-call rotation on occasion.
  • A minimum of two years' experience, at least one of which is in public health, home care, or hospice nursing, is preferred.
  • Self-directed and able to work with minimal supervision.
  • Demonstrates excellent observation, problem-solving, verbal and written communication; nursing skills per competency checklist.
  • Shows the ability to organize and prioritize workload independently.
  • Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
  • Full-time employees accrue up to 168 hours of PTO annually, in addition to sick time accrued at the rate of 1 hour for every 40 hours worked. Bereavement and jury duty leave are also available.
  • Option to enroll in medical, dental, vision, and life insurance. Heartlinks pays for 100% of the employee's medical premiums and 75% of the employee's dental and vision premiums. Health benefits begin the first of the month after the start date.
  • Miles driven for work are reimbursed.
  • Holiday pay for 6 observed holidays.
  • Ability to enroll in 401(k) and begin contributing at the time of hire. Heartlinks will match dollar for dollar up to 5% of earnings after one year of employment and 1080 hours worked.
  • Premium pay for hospice-related certifications.
