Cynet HealthStaff is seeking an experienced RN Case Manager in Alaska for a full-time day shift. You will perform comprehensive patient assessments, develop individualized plans of care, administer medications, and coordinate with multidisciplinary teams to ensure quality and safe patient outcomes.

The role requires a current, unrestricted RN license, a minimum of 2 years' nursing experience, and BLS certification; Case Management certification is preferred.

Description:The Registered Nurse (RN) provides professional nursing care across the lifespan in accordance with organizational standards, policies, and evidence-based practice.The RN utilizes the nursing process to assess, plan, implement, and evaluate patient care while delivering high-quality, culturally sensitive, and patient-centered services.The RN collaborates with providers and interdisciplinary teams to promote optimal patient outcomes, safety, and positive patient experiences.

Key responsibilities include:Performing comprehensive patient assessments and developing individualized plans of care.Implementing and evaluating nursing interventions and patient responses.Administering medications and treatments safely and accurately per provider orders and policy.Recognizing changes in patient condition and initiating appropriate interventions or escalation of care.Providing patient and family education related to care, medications, and discharge needs.Delivering culturally responsive, patient-centered care.

The RN independently applies critical thinking and professional judgment to:Perform and interpret comprehensive assessments and recognize significant changes in patient condition requiring nursing intervention or provider notification.Exercise independent nursing judgment in determining appropriate nursing actions, interventions, and priorities.Develop, implement, evaluate, and revise individualized nursing plans of care based on patient needs, preferences, and clinical circumstances.Integrate clinical findings with ethical principles, cultural considerations, patient safety standards, and regulatory requirements when making nursing decisions.Accept accountability for nursing decisions, delegated tasks, and outcomes of nursing care in accordance with organizational policy.

The RN collaborates with providers and interdisciplinary team members to coordinate care.Facilitating referrals, follow-up care, and continuity of services is essential.Documentation of assessments, interventions, and outcomes must be accurate and timely.Advocating for patient care and assisting patients in navigating the healthcare system is crucial.

Practicing within the nursing scope of practice, the RN adheres to regulatory, safety, and infection prevention requirements.Participating in quality and performance improvement activities is expected.

The RN demonstrates professionalism, accountability, flexibility, and teamwork.Providing appropriate guidance and delegation to ancillary staff is part of the role.Supporting orientation, mentoring, and education of staff, students, and peers is important.Floating to other departments or assignments as needed may occur.

Knowledge, skills, and abilities required include:Knowledge of nursing principles, evidence-based practice, medication safety, and infection prevention.Strong clinical judgment, organization, prioritization, and communication skills.The ability to work independently and collaboratively in a fast-paced environment.Proficiency with electronic health records and clinical systems is necessary.

Physical work requirements include:The ability to lift, push, pull, and move up to 50 pounds.The ability to stand for extended periods and perform bending, stooping, and kneeling.The ability to work in a moderate-to-high noise clinical environment.
