Job summary

WakeMed is seeking an experienced RN Supervisor to lead the Population Health Care Management team. The role focuses on developing care managers, advancing population health strategies, and delivering patient-centered care across the continuum.

The supervisor partners with providers and interdisciplinary teams to improve outcomes, coordinate services, and support value-based care initiatives. Strong leadership and change-management skills are essential.

Qualifications

• Registered Nurse with current licensure and leadership experience in care management.
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification required.
• Experience leading clinical teams in population health or care management settings.

Responsibilities

• Provide leadership, coaching, and supervision to the care management team.
• Oversee daily care management operations and collaborate with leadership to meet quality goals.
• Serve as clinical expert on care management, transitions, and population health best practices.
• Lead initiatives to improve patient outcomes and reduce avoidable utilization.
• Foster partnerships with primary care, inpatient teams, and community resources.
• Monitor KPIs and implement process improvements as needed.
• Guide teams through organizational growth and evolving healthcare delivery models.
• Identify barriers to care and coordinate across settings to optimize transitions.
