Job summary

Providence at Home with Compassus seeks a Clinical Care Partner to coordinate safe, efficient transitions of care for hospitalized patients. You will evaluate patients for home-based services and support timely discharge planning with physicians, case management, patients, families, and post-acute providers.

Qualifications

• RN license active and unrestricted.
• Experience in care coordination or discharge planning preferred.
• Experience with EMR systems (Epic) and referral platforms preferred.

Responsibilities

• Coordinate discharge planning and transitions to ensure timely, safe care.
• Educate patients, families, and staff on post-acute options and pathways.
