Job summary

Providence is seeking an RN Care Transition Coordinator to provide care transition services for hospitalized patients. Minimum qualifications include graduation from an accredited nursing program and WA RN license; preferred Bachelor’s in Nursing and 1 year clinical experience with experience

Description

The RN Care Transition Coordinator position is responsible for providing care transition services for Providence hospitalized patients. The Coordinator works with the Providence physician team and hospital discharge planning staff to ensure patients are successfully discharged from the hospital with no disruption in services. The position encompasses the roles of nursing assessment, liaison abilities, communication, education and team/family support.

Required Qualifications

• Coursework/Training - Graduate from an accredited school of Nursing.
• Washington Registered Nurse License upon hire.

Preferred Qualifications

• Bachelor's Degree in Nursing.
• 1 year Clinical experience.
• Experience in providing patient and staff education.
• Experience in conflict resolution.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
